Social Services

The decision to enter a nursing home is often difficult, and one that must be supported. The Social Work staff at Pleasant Ridge Manor begins working with residents and their families prior to admission to ease the transition to the nursing home. This support is continued throughout each resident's stay. In keeping with our goal of helping each resident reach and maintain their highest level of well-being, the social work staff delivers the following services:

  • Helping residents become comfortable and adjust to the nursing home
  • Providing and arranging for a variety of counseling services
  • Helping residents determine and declare their health care decisions
  • Linkage to community resources
  • Assistance in resolving concerns
  • Discharge planning as appropriate to assure a successful return home

 

Clinic

The clinic, utilized by residents of PRM East and West, provides a specialized area for medical consultations by our specialty physicians, and is similar to a large doctor's office. The clinic also coordinates and provides transportation to necessary outside medical appointments, such as those for renal dialysis, chemotherapy, and radiation.

Additionally, the clinic aids residents with glasses adjustment and repair, hearing aid fittings, and the purchasing and fitting of new shoes, all of which help facilitate comfort.


Pharmacy

Pleasant Ridge Manor maintains an in-house pharmacy, which prepares medications utilized within the facility. It is staffed by two full time pharmacists with years of experience in geriatric pharmacy. Qualified pharmacy technicians also assist with filling prescriptions.

The pharmacists work closely with the physicians, nurses and other health care professionals to ensure the best in pharmaceutical care for each resident. Each new medication order is monitored for drug-drug; drug-food, drug-lab interactions or any other drug related problems. A pharmacist review of all medications for each resident is done monthly. These reviews are done with the goal of maintaining or improving the quality of life for each resident.


Dietary Services

The staff of Pleasant Ridge is committed to meeting the daily nutritional needs of our residents through a mealtime program that is not only healthy and therapeutic, but enjoyable, too.

Our dietary services are managed by an experienced foodservice staff. Three full-time registered dietitians monitor the nutritional needs of every resident, and accommodate physician-directed special diets.

We understand that mealtime is an important part of the day, and thus offer a varied menu of tasteful, attractive meals, served by friendly, helpful staff. Residents are asked about their food preferences upon admission, and are asked to participate in the "Resident Food Committee", a resident group which offers input into the daily menu as well as planning menus for special events and holidays. Special events such as Hawaiian luaus, Happy Hours, and summer barbecues add to the special pleasure of our mealtime experience.

PRM also invites residents' family members to attend Sunday Brunch on Mother's Day and at Christmas time. This complimentary meal allows family members to spend special days with their loved ones in a relaxed, family-style setting.
